A "Colorado Beetle" empties your poo without the use of an outhouse. Presumably your poo, normally a semi-solid, combines with the chemicals found in the crunched-up beetle and the whole mess vaporizes. This vaporization costs you 10 health before the beetle's health benefits kick in.
