Once you have a personal island set up, your "Island Hawk" button will be enabled. A "Stargate", sometimes just called an "Icon", is created where a hawk is used.
Hawk/Stargate Icon Use
Stand on a bare square next to you then click the hawk button found next to the Stop button (View>Stop). Instantly 2 things happen: your hawk will fly you to your island and a stargate-looking icon will be created on the square on which you were standing. EXCEPTION: your hawk will be too scared to take you to your island if you have chopped on any fencing in last few minutes.
The subsequent use of your hawk will transport you back to where your stargate was created, destroying the stargate. If this subsequent use is on your island, using the hawk on a bare square is necessary. If someone destroys your stargate by chopping it with an axe, it is considered a subsequent use and you are transported back. There is less than a minute wait before you can use your hawk again to return to your island.
This is similar to using a wood duck but since your hawk has no birdhouse to return to, they are not anxious about returning anywhere within a certain time limit.
Considerations
Mousing over a stargate indicates: its coordinates, Island Name (Player ID: #####).
Creating a stargate somewhere where it can be fenced in is dangerous, particularly if you are taking a long trip to your island or plan to log out while on your island. Keep in mind that your stargate remains even after you become inactive.
There is a wait timer when using your island that is dependent on the number of times you have used your island that day, plan accordingly.
Unlike other birds, hawks do not need to be fed. They would not care for worms, anyway.
Historical Notes
For a while when hawks were first introduced to Toadwater, the hawk was an actual inventory item. Many of these hawk items still exist in game (they are sort of considered early-adopter/members-only collectors items).
